JOB SUMMARY

The LVN utilizes the nursing process to assess and identify the individual needs of patients through observation of disease symptoms, emotional state, laboratory reports, cultural and socio economic background and response to medical regimen and treatment. The position will report to the practice manager and clinically under the direction of the RN manager, s/he will assist with including but not limited to: medication prep, medication authorization, patient coordination, review of new patient referrals, triage provider in basket messages, and assist with rooming patients as needed.

This individual will utilize his/her good customer service and communication skills on a daily basis via phone/electronic contact, mail correspondence, and personal contact with both internal and external customers.  This employee will utilize computer skills in ApeX and other UCSF computerized programs. This individual supports the standards of the policies and procedures of the practice, and is responsible for maintaining current knowledge of pharmacology and competency in modes of administration, and complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, and administrative and clinical practice.  S/he is required to adhere to the Telephone and House Standards as adopted by the Medical Center, as well as demonstrating UCSF’s PRIDE values and the AIDET fundamentals of communication in all patient care and stakeholder interactions.

Department Description
Dealing with an illness can be a very difficult time for patients and their loved ones. At UCSF Health, experienced and sensitive social workers in our Department of Social Work are available to assist patients and their families cope with the many psychological and social issues that may arise when facing illness, hospitalization and medical treatment. Our social workers act as patient advocates by serving as counselors, helping to find resources, and solving problems. Any concerns related to the psychological, practical, and social well-being of each patient or their loved ones may be discussed openly with our social workers.
Qualifications
Required qualifications: - Licensed as a Vocational Nurse in the State of California - Minimum of 3 years of experience as an LVN in an ambulatory clinic. - Knowledge and abilities essential to the successful performance of the duties assigned to the position, including demonstrated excellent communication skills, problem-solving ability, and high comfort level with learning new technology/systems. - Basic computer/keyboarding skills. - Prior experience with electronic systems/records. Preferred qualifications: - Recent LVN experience in a Neurology clinic - Recent experience in Epic EHR - Recent experience medication reconstitution Licenses/Certifications: - Licensed as a Vocational Nurse in the State of California - Current American Heart Association BLS Certification
