Clinical Social Worker I/II

Under the supervision of the Citywide Focus Kujichagulia Program Director, the incumbent provides continuous, comprehensive mental health services, including crisis intervention, outreach, therapy, and case management, to adults with serious mental illness who are high utilizers of psychiatric and behavioral health services and who often have co-occurring substance use disorders and/or chronic medical conditions. The Kujichagulia team provides culturally responsive, recovery-oriented services with a primary focus on African American clients.

The incumbent assesses presenting problems; obtains psychosocial histories; conducts comprehensive mental status examinations; and formulates diagnoses in accordance with the Diagnostic and Statistical Manual of Mental Disorders (DSM-5-TR). The incumbent develops, implements, and monitors individualized treatment plans; provides individual, group, and family interventions, as appropriate; participates as a member of a multidisciplinary treatment team; collaborates with psychiatrists, nurses, peer specialists, and other behavioral health professionals to coordinate client care; serves as a liaison with inpatient psychiatric units, community-based organizations, and other service providers to facilitate referrals, care transitions, and continuity of treatment; provides ongoing case management, crisis intervention, and care coordination services; advocates for clients in accessing behavioral health, medical, housing, vocational, and other supportive services; maintains accurate, timely, and confidential clinical documentation in accordance with California state law, City and County policies, Medi-Cal documentation standards, and departmental requirements; participates in quality improvement and utilization review activities, as assigned; attends required staff meetings, clinical supervision, and professional development trainings; maintains compliance with applicable professional licensure, certification, and continuing education requirements; and performs other related duties as assigned.

 

MINIMUM REQUIREMENTS: - CSW I: Masters degree in social work, psychology, or counseling or related field from an accredited school of social work or equivalent experience/training and knowledge and abilities essential to the successful performance of the duties assigned to the position - CSW II. A Master's degree in social work, psychology or counseling from an accredited school of social work and possession of the requirements necessary for application for licensure as a clinical social worker, as determined by the California Board of Behavioral Science Examiners; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position - Ability to work cooperatively and communicate effectively with others. Ability to work in a high stress, high volume area and work well under pressure and with deadlines. Ability to follow oral and written instructions accurately, and be detailed oriented. Able to efficiently prepare written summaries of activities, set priorities and complete reports within required timeframes. Knowledge of medical and/or psychiatric terminology - Excellent communication and interpersonal skills - Able to work as a team member in a culturally diverse setting - Able to lift 50lbs and climb stairs - Demonstrated excellent attendance and reliability - A preliminary job offer and assignment may be extended pending successful background clearance. - An understanding of mental health issues and sensitivity toward mental health clients - Ability to work with clients and program staff from diverse cultural and social class backgrounds PREFERRED: - Two years working with severely mentally ill adults, the homeless and dual diagnosed populations - Case Management and Care Coordination experience - Culturally competent in working with clients from diverse cultural backgrounds and adverse lived experience. - One year experience working with African American clients - One year experience doing community-based case management and the ability to do outreach including to SRO hotels - CSW II: Eligible to test for licensure with the Board of Behavioral Science as An Associate Clinical Social Worker (ACSW) or Intern Marriage Family Therapist (IMFT). LICENSES/CERTIFICATIONS: - CSW II.
