The Division of Geriatrics seeks a per-diem Nurse Practitioner to support the Age-Friendly Geriatrics Emergency Department (AFED) consult service. UCSF is engaged in providing care based on “What matters” as part of the Age-Friendly Health System (AFHS). This position requires optimizing the care of older adults presenting to the emergency department setting when geriatric consultants are requested by ED providers. The clinical work involves providing consult services from Geriatrics to ED by completing structured assessments, patient exam, and making recommendations to the ED and Primary Care Physicians (PCPs) for ongoing care/ home management as indicated. It involves coordination with pharmacists, ED providers, geriatricians, social workers, community-based providers, and other interdisciplinary professionals and/or services to create an optimal discharge or ongoing transition of care for the patient. The position requires working collaboratively with the geriatricians in the Division of Geriatrics that are identified on scheduled shifts, and with the ED attendings and providers. The position DOES NOT involve direct clinical management responsibilities of patients in the emergency department. The per diem position requires commitment of at least 4 shifts per month and all shifts are 8-10 hours with onsite clinical coverage 7 days per week. This translates to approximately 20% FTE per month, not to exceed 40% FTE per month.
Our ideal candidate is a practicing nurse practitioner who possesses the advanced education, knowledge, clinical skills, and experience necessary to provide expert advanced nursing consultation in the geriatric specialty. Under the supervision of Geriatrics attending and guided by UCSF Health policies and procedures, the APP position offers a broad scope of practice in providing consultative services within a care pathway using multiple assessments and medical knowledge in the geriatric specialty in settings that involve coordinating with professionals managing chronic stable medical conditions, specialty protocols, routine care, acute or critical care per approved standardized procedures. The APP will provide high-quality patient-centered care to adult and geriatric patients at Parnassus campus but may involve consultation across all UCSF Health licensed settings.
